Falsify

vb. 1. To make something false; to counterfeit or forge <the chiropractor falsified his records to help the plaintiff>. See COUNTER. FEIT; FORGERY. 2. Rare. To prove something to be false or erroneous <their goal in the appeal was to falsify the jury's verdict>.

falsifying a record

The crime of making false entries or otherwise tampering with a public record with the intent to deceive or injure, or to conceal wrongdoing. 18 USCA §§ 1506, 2071, 2073; Model Penal Code § 224.4 (1997).

surcharge and falsify

To scrutinize particular items in an account to show items that were not credited as required (to surcharge) and to prove that certain items were wrongly inserted (to falsify).
